  • Mytherapeace Global, PC (“Professional Corporation”) is required by law to maintain the privacy and security of your Protected Health Information (“PHI”). We will notify you promptly if a breach occurs that may compromise the privacy or security of your PHI.

  • We may use or share your information to:

    • Provide, coordinate, or manage your treatment

    • Bill and receive payment for services

    • Conduct healthcare operations, including quality improvement and compliance

    • Comply with legal obligations

  • You have the right to:

    • Access and obtain a copy of your medical record

    • Request corrections to your information

    • Request confidential communications

    • Request restrictions on certain disclosures

    • Receive an accounting of disclosures

    • File a complaint if you believe your privacy rights were violated

  • We will obtain your written authorization for uses and disclosures not described in this Notice, including most uses of psychotherapy notes and certain marketing communications.

  • We may share PHI with trusted service providers who perform functions on our behalf and who agree to protect your information in accordance with applicable law.

  • If you believe your privacy rights have been violated, you may file a complaint with My Therapeace Global, PC or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
